　[医学系のみ]
※医学系の大学院（医薬保健学総合研究科医学博士課程，医科学専攻（修士課程）並びに先進予防医学研究科）学生は，１年次後期（４月入学者のみ）及び２年次前期以降の授業料免除申請の際，併せて提出してください（保健学，薬学，創薬科学専攻の学生は提出不要）。

※[A]在学者は，本様式1-1，1-2ではなく，Web申請入力が必要です。Web申請入力の上，ダウンロード・印刷した本人調書を提出してください。

  • Declaration of the value of damage to housing and property resulting from disasters (to be submitted if the applicant has suffered a disaster such as a windstorm or flood within the year prior to the application)【English】

 

　[Medical students only]

• • 授業料免除申請大学院学生推薦調書*Japanese text only

Students of the Graduate School of Medical (Doctoral Course in Medicine, Master’s Course in Medical Sciences) and Graduate School of Advanced Preventive Medical Sciences, they must submit this form “授業料免除申請大学院学生推薦調書” when applying for tuition fee exemption at the following time.
・In the second semester of the 1st grade (Only students who enroled in April)
・From the first semester of the 2nd grade onwards(All Studens).
*No need for Students of School of Health Science, School of Pharmacy, School of Pharmceutical Sciences.

 

[B]only new students

※[A] Current students must apply through the web-based application system rather than using Forms 1-1 and 1-2. Upon completion of the online application, please submit a downloaded and printed copy of the Personal Information Form.
